Clients have the right to file a complaint regarding professional services.

To file a consumer complaint, you may contact the Texas Office of the Attorney General (OAG), Consumer Protection Division using the link below:

This process allows consumers to submit concerns or complaints related to professional services in Texas.

Clients may request copies of their health care records directly from this practice.

To request records:

1. Submit a written request by email to COUNSELORERICAJONES@GMAIL.COM
   OR

2. Submit a request through the secure client portal.

 

Requests will be processed in accordance with Texas law and HIPAA regulations.

 

Identity verification may be required prior to release of records.
